your UHMC.
Then, select Request Service -> Hardware -> Mobile Device -> Request New
Phone and fill out the form.

How do I report a lost work phone?

Please go to https://sbm.cherwellondemand.com/ and sign in with your UHMC.
Then, select Request Service -> Hardware -> Mobile Device -> Lost or Stolen Device and fill out the form.

How do I get help if something is wrong with my work phone?

Go to https://sbm.cherwellondemand.com/ and sign in with your UHMC. Then, select Report Incident-> Hardware -> Mobile Device -> Assistance with iPhone

How do I request temporary international coverage for my work phone?

Go to https://sbm.cherwellondemand.com/ and sign in with your UHMC.
Then, select Request Service -> Hardware -> Mobile Device -> International Travel Request and fill out the form.

How do I request accessories for my work phone? (EX: Charger, case, screen protector)

Then, select Request Service -> Hardware -> Mobile Device -> Request Accessories and fill out the form.

How do I make an appointment to come to the IT office?

Put in the appropriate ticket for your situation. A technician will reach out to you to schedule an appointment via email unless you specify otherwise on the ticket.

Where is the IT office?

The IT office is located in HSC Level 3 Room 121. If you take the escalators down from level 5 of the hospital and make a right, it's down the second hallway on the left.
